SIMTIAR NEXT STEPS · AGES 18–26

Post-Majority & TAP Support.

Build confidence for adult life, one step at a time.

Our Transition to Adulthood Program provides a flexible pathway from care toward greater independence. Preparation can begin in Simtiar Services Group Living program, with support adjusted to the person’s goals, needs and readiness. Trauma-informed transition planning gives young people a voice, prepares them for changes and maintains trusted relationships along the way.

Three phases. Your own pace.

Age provides guidance. Progress considers the person’s wishes, skills, health, safety, housing, funding and support needs.

01

Preparation

Age guide: 15–18 · Group Living

Discover strengths and future goals. Practise cooking, household routines, budgeting, transport and self-advocacy. Explore housing, education, work and adult services.

Working toward: An Individualized Transition Plan, everyday confidence and strong family, cultural and community connections.

02

Launch

Age guide: 18–21 · Adult living

Practise adult responsibilities with structured coaching. Support can include housing, tenancy, income applications, banking, appointments, transportation, education and employment.

Working toward: Stable housing, access to adult services and greater confidence in everyday decisions.

03

Independence & growth

Age guide: 21–26 · Ongoing growth

Sustain housing, health and financial stability. Pursue education or employment goals and strengthen relationships, with periodic coaching where needed.

Working toward: Lasting stability and a planned transition from formal support when safe and appropriate.

Readiness guides progress. A person may remain in a phase longer, work across phases or temporarily return to more support. Age alone does not decide their next step.
